Housing Case Manager - East New York

The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to a caseload of residents who are living with HIVand/or and co-occurring conditions. The Housing Case Manager works closely with their clients in the clients’ homes, in the office, and in the community helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness.
