Intake Coordinator Registered Nurse (RN), PACE Program, Full-time

Job Summary Working independently, the PACE Intake Coordinator, RN schedules intake appointments, coordinates required home and center visits, documents prospective participant medical needs, and gathers essential medical and social information throughout the enrollment process. The coordinator may also provide tours of the PACE Center as part of participant and caregiver orientation.
