About the role
Mozaic is hiring for a Skills Coordinator! Skills Coordinator Duties and Responsibilities: • Assists in the development, implementation and review/revision of written service plans for individuals. • Implements activities and ongoing services to meet outcomes/goals of each individual served based on interests needs and choices. • Acts as the on site supervisor in the absence of the Site Supervisor. • Provides support and oversight for individuals in activity of daily living and community integration. • Advocates for individuals served to protect their rights, meet their needs and increase their independence and well being. • Develops written outlines/goals that are clearly stated and measurable, establishes completion criteria and develops step by step methods for implementation and completion with target dates. • Administers medications and other nursing services as needed under the direction of a Registered Nurse. • Provides individuals assistance on activities of daily living such as grooming, cooking, bathing, toiletings, communication and others as needed to promote and improve independence.
