We are looking to immediately hire a Part time (30 hours/week) or Full time (40 hours/week) Occupational Therapist Includes a $5,000 Signing Bonus for Full time, prorated if you work Part time! to assist us to help seniors help themselves and allow them as much independence as possible! As an OT with Kline Galland Home Health your responsibilities will include: Providing initial and ongoing assessment related to physical and functional status, home safety, as well as cognitive, psychological and perceptual function to determine baseline and determine appropriate, achievable goals. Developing and revising the plan of care in collaboration with patient/family/caregiver and the other care-team members. Responsible for planning, evaluation, and treatment implementation of assigned caseload. Preparing clinical progress notes. Supervising and home health aides as appropriate. Evaluating outcomes. Planning Discharge. Educating patients and/or families about proper safe use and recommend methods for installation of adaptive equipment. Evaluating outcomes. Planning Discharge.
