Responsibilities include: Managing an assigned hospice caseload throughout Gadsden and Liberty Counties Completing comprehensive nursing assessments and developing individualized plans of care Coordinating care with physicians and the interdisciplinary team Managing pain and symptoms to maximize comfort and quality of life Educating patients and families about disease progression, medications, and available resources Supervising Hospice Aides and LPNs assigned to your patients Documenting patient care accurately in the electronic medical record Building trusted relationships with patients, families, healthcare providers, and community partners These responsibilities reflect the RN Case Manager role, including coordinating the interdisciplinary plan of care, managing hospice patients, supervising Hospice Aides and LPNs, completing assessments, providing education, and ensuring quality care.
