RN Care Manager - Summit Medical Group

Summit Medical Group is recruiting for an RN Care Manager. This is a full time opportunity and after successful completion of introdcutory period this is a hybrid office/work from home position. This position provides individualized care to patients with complex, chronic medical conditions designed to ensure health care continuity for SMG patients.
