School Based Community Case Manager

School-Based Case Manager (High School Focus) Shape the Future, One Student at a Time. Adolescent support changes lives. We are looking for a dedicated Case Manager to join our team at McPherson High School. In this role, you will support high school students (ages 14–18) by providing community-based mental health services that focus on youth strengths, life-skills development, emotional regulation, and preparing for adulthood. What You’ll Do: Support Growth: Help students and their families identify their strengths to improve self-esteem, coping mechanisms, and behavioral outcomes. Teach & Model: Provide skill-building training to help students navigate peer dynamics, manage stress, build independence, and establish healthy routines. Crisis & Advocacy: Be a steady support during a crisis and an active advocate for the student’s mental health needs within the school system. Clinical Partnership: Work closely with a qualified mental health provider to implement and track progress on individual treatment plans.
