About the role
You will partner with Enhanced Care Services Director to develop, execute, monitor and evaluate all Case Management activities. The Case Manager Team Manager will provide health education and care coordination services across multiple health care settings to high-risk patients, as well as provide onsite supervision of Case Managers and Case Managers 1.
