We seek a Value Based Care Physician Leader to support a growing team in Houston. As a leader, you will partern with other Physician Leaders and Regional Leaders to drive HEDIS and quality-focused patient outcomes with a provider team who manage the chronic conditions of our high-risk Medicare patients Monday - Friday.
