What You'll Do Complete routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing facilities. Assess changes in condition and provide expert pain and symptom management. Deliver wound care, medication education, and comfort-focused nursing interventions. Support patients and families through compassionate communication and education. Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team. Complete timely, accurate EMR documentation. What You'll Bring Active Minnesota Registered Nurse (RN) license. Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, or end-of-life care. Strong assessment, communication, and clinical decision-making skills.
