Job Summary: Come with work a cohesive, supportive team in the CCS GROW program! The Behavioral Health Care Coordinator position works within the CCS program for Dane County, providing robust and comprehensive care for people with mental health and/or substance use recovery. This includes community based direct services with clients and their systems, creating individually tailored recovery plans, assessments, and services to fit their needs.
