Job Summary: As a Rehabilitation Case Manager with our Adult Outpatient program, you'll work with adults of all ages, providing support for their mental and emotional health in community-based settings. The goal is to help them feel better emotionally and manage different parts of their lives more successfully. You’ll work with a team to provide support to individuals like helping them learn new skills, figuring out what kind of help they need, and making plans for their care. You'll do all of this while being guided by a licensed behavioral health professional. Essential Duties/Responsibilities: Complete intakes into services. Assess client needs related to multiple areas of their lives. Create personalized care plans with clients and clinicians to support recovery and build skills. Provide case management and connect clients with helpful resources to improve daily life. Deliver skills training (individual, group, family), coaching, and case management as outlined in the care plan. Communicate regularly with other care providers to coordinate client support. Lead group outreach for specific underserved groups (e.g., unhoused young adults, LGBTQ+ young adults) as needed. Complete all required client documentation in the electronic health record following agency and county standards. Meet the minimum for billable service time (45%). Manage your own schedule to effectively balance client appointments and other tasks during your work hours. Provide services in the community, including traveling to clients' homes, schools, or other locations. Attend required meetings and training. Participate in activities to improve quality and safety. Keep the work area clean and report any maintenance or safety concerns immediately. Regular attendance is essential for this job. Additional duties as assigned.
