W hat You'll Do:

The Case Manager III facilitates admission intakes, treatment planning, discharge planning, obtaining collateral, clinical interventions, crisis management, assists in the legal process and initiates personalized care for individuals who are in crisis and stabilizing from crisis. Works collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for Individuals, their family members, clinics, local providers, and/or other referral sources. This position will carry a caseload.

  • Conducts a thorough, accurate, clinically sound Psychosocial Assessment with the individual in a timely manner.
  • Ability to review and sign off on Psychosocial Assessments completed by Certified AAC or associate level staff members, if fully licensed.
  • Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
  • Participates regularly in Interdisciplinary Treatment Team meetings.
  • Collaborates with the Treatment Team to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Complete the signed Treatment Plan and review with each individual and any other involved parties.
  • Gather collateral from the individual and involved parties to provide a thorough assessment of an individuals' needs for treatment and discharge.
  • Maintain a working knowledge of the Involuntary Treatment Act (ITA) and Evaluation and Treatment (E&T) Involuntary Commitment process. Monitor the petition process to ensure timeliness are adhered to, and forms are complete and thorough. When subpoenaed, attend E
