Care Coordinator, Population Health Services

Reporting to the Director of Population Health, the Community Care Coordinator will work collaboratively with the Population Health team to assess patients' social, emotional, and financial needs, develop and implement treatment plans, and connect patients with resources to support their overall well-being and recovery. The ideal candidate will assist in directing and supervising a connected team of Community Health Workers (CHWs) to provide patient panels and engage in follow-up and treatment to mitigate social determinants of health (SDoH) that present as barriers to care with the goal of optimizing the overall health of the population of service.
