PATIENT NAVIGATOR | AMBULATORY CARE SPECIALITY - NEUROLOGY| FULL-TIME DAY SHIFT (25512)

The Patient Navigator is a primary point of contact for patients seeking care within the Ambulatory Services Network. The Navigator will support patients and their families with educational resources about their diagnosis and helps to navigate the many paths of a healthcare diagnostics and treatment process. Navigators screen patients for additional psychosocial needs and will provide referrals and linages to services provided within Bergen New Bridge and to community resources where appropriate.
