Hospice LPN (Licensed Practical Nurse) - Full Time Location: Tiffin & Fremont, Ohio Employment Type: Full Time Join Our Hospice Team We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice team serving patients in the Tiffin and Fremont communities. This role provides an opportunity to make a meaningful difference by delivering high-quality nursing care and support to patients and families during their hospice journey. As a Hospice LPN, you will work collaboratively with an interdisciplinary care team to ensure comfort, dignity, and exceptional patient-centered care. Responsibilities • Provide skilled nursing care to hospice patients in accordance with the plan of care. • Monitor patient conditions and report changes to the RN Case Manager and clinical team. • Administer medications and treatments as ordered. • Document patient care accurately and timely within the electronic medical record. • Educate patients and family members regarding disease processes, medications, and comfort measures. • Collaborate with physicians, nurses, social workers, aides, and other members of the hospice team. • Support patients and families with compassion, empathy, and professionalism. • Participate in care conferences and interdisciplinary team meetings. Qualifications • Current Ohio Licensed Practical Nurse (LPN) license in good standing. • Graduate of an accredited practical nursing program. • Previous hospice, home health, long-term care, or medical-surgical experience preferred. • Strong assessment, communication, and documentation skills. • Ability to work independently while maintaining excellent teamwork. • Valid driver's license, reliable transportation, and acceptable driving record. Preferred Qualifications • Hospice or home health experience. • Experience working with electronic medical records. • Knowledge of end-of-life and palliative care principles.
