About the role

GENERAL POSITION SUMMARY : The Case Manager provides onsite case management and coaching services to participants in the program and follows up with each client throughout their stay work to provide the wrap-around services from admission to discharge. The Case Managers documents client demographic information, all interactions with clients and outside providers in a timely manner, including maintaining accurate, up to date client files. Knowledge, Skills, and Abilities : Ability to understand the challenges faced by homeless singles or families. Ability to interact effectively with homeless singles or families. Excellent written and verbal communication skills are a must. Effective time management skills. Handle multiple tasks, coordinate with supervisor, and prioritize needs. Capacity to identify client information into case files and compile statistics. Ability to work independently and work with the Program Manager to assure quality of services provided. Ability to use computer to enter client information into case files and compile statistics. Knowledge of Suffolk County resources and experience navigating Social Service Agencies helpful. Aptitude to assist client in identified barriers. Knowledge of SOARS and HMIS The - Case Manager - Housing Division main job duties are the following: Ensure the safety of all clients in the shelter. CM MUST address ALL safety related issues immediately. Assists clients in achieving self-sufficiency through advocacy, resource referrals & assistance with housing and job placement. Meets with clients to establish goals & action steps towards those goals & to monitor progress. Develops and maintains appropriate records, statistics and documentation of services to the clients. Advocates with the clients when they have barriers to overcome in meetings their goals. Negotiates with landlords and other service providers to ensure stable housing. Maintain appropriate professional boundaries with all clients. Attend monthly staff meetings and actively participate in weekly one-on-one with supervisor. Document and oversee any and all client and facility emergencies; Complete intakes on all clients placed in shelter; Complete and discuss shelter intake, ILP and ILP Violations, progress notes, face to face with clients. Address all residents' compliance issues when they occur; Complete school transportation paperwork to facilitate children attending school; Maintain communication with SCDSS casework staff on infractions and provide documentation; Maintain confidentiality of the client's personal information; Complete SPA application for those client meeting criteria; Serve as a representative on behalf of agency Perform other tasks as delegated by supervisor/program manager Complete bio-psychosocial history of clients; Perform and supervise fire drills as directed by Program Manager; Attend supervision sessions with Program Manager; Assist clients to maintain responsibility for themselves, their children, and environment Assess clients' needs and make appropriate referrals; Counsel clients and refer to outside agencies for more in depth counseling needs; Review and accepts housing search logs for residents and submit to SCDSS monthly; Assist Program Manager in training new staff members. Vocational and House Assistance Compete SOAR, SMS, HMIS and other funding source's tracking Complete any and all other tasks assigned by supervisors/Program Manager EDUCATION and/or EXPERIENCE Bachelors college degree preferred, (preferably in a related behavioral science field) Three years of supervisory experience Ability to become competent in implementing Halo Network's philosophy, policies and procedures to best meet the treatment needs of the client population. WORK ENVIRONMENT: The standard 40 work-week with an 8 hour shift applies. Some overtime may be required during the planning and execution of special events. RELATIONSHIP: REPORTS TO DEPARTMENTAL HEADS.
