Population Health Navigator

High School diploma or equivalent; CMA or LPN certification required. The PHN collaborates closely with providers, care managers, and other interdisciplinary team members to address clinical and non-clinical needs, coordinate services across the care continuum, and support patients in achieving self-management goals. This position requires strong communication skills, clinical knowledge, and a commitment to improving the health of targeted patient populations.
