Licensed Vocational Nurse Home Health LVN

Summary of Essential Functions for the Position: A qualified Licensed Vocational Nurse (LVN) administers one-on-one skilled nursing care to patients on an intermittent basis in their place of residence. The nurse collaborates with physicians, patients and their families, nurses, therapists, social workers, and other members of the patient care team to implement and follow a patient specific care and treatment plan in accordance with the physician orders and under the direction of the Registered Nurse. The nurse will assess, monitor, document and report progress of patients’ health and condition using required clinical notes in the clinical technology within established timelines. Services are furnished in accordance with Agency policies.
