- Responsible for advocating for and with our members to ensure informed decision making, informed consent, and appropriate guardianship. - Responsible for scheduling, leading and actively collaborating with our members and their interdisciplinary team to conduct meetings and assessments ensuring the development of a comprehensive, person-centered Life Plan that reflects the person’s needs and desired life goals. - Implement, update, and monitor Life Plans and facilitate individualized Life Plan reviews and approval processes. -
