Director, Case Management

The Director of Case Management provides strategic and operational leadership for Case Management, including Nursing Care Transition and Social Work functions across SMH. This leader is responsible for ensuring effective care coordination across the continuum, optimizing patient throughput, supporting quality outcomes, regulatory compliance, and financial stewardship while promoting exceptional patient and family experiences.
