Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our growing Home Health team in Ventura County . This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.

As a Home Health Physical Therapist , you will work with senior care populations , including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings , helping patients regain independence and improve their quality of life.

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health , you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients .

Key Responsibilities:

  • Conduct in-home physical therapy evaluations and reassessments
  • Develop and implement personalized rehabilitation care plans
  • Improve strength, balance, mobility, and functional outcomes
  • Provide therapy in home health, assisted living, and community care settings
  • Educate patients and families on fall prevention and safe mobility techniques
  • Document visits using WellSky (Kinnser) EMR
  • Collaborate with interdisciplinary home health teams

Qualifications

  • Licensed Physical Therapist (PT) – California (Required)
