The didgwalic Wellness Center is seeking a dedicated Registered Nurse Care Manager to join our integrated care clinic that combines primary care, behavioral health, and opiate treatment all in one place. The RN Care Manager serves as an advocate for an individual’s healthcare needs and is dedicated to enhancing the health status of the community. Although focused on high-risk patients, the working RN is a part of the overall medical team. Duties/Responsibilities: Partners with Providers, LPNs, and Medical Assistants in client/patient care. Assists with the identification of patients with special health care needs/chronic diseases. Develops a method of tracking patient compliance with clinical guidelines and the plan of care. Uses an empathetic and positive approach to engage clients in shared decision-making and in collaboration to develop care plans. Ensures patient confidentiality as defined by HIPAA and the medical facility. In partnership with didgwalic Leadership, develop objective measures of performance with a clear process for implementing CQI (Continuous Quality Improvement). Develops, conducts, and evaluates formal or informal in-services and programs with a focus on the highest risk groups Offer one-on-one educational support for each risk group or an individual patient, tied to best practice methodology, Addiction Care Models, and trauma-informed care. Partner with all departments to ensure that clients’ needs are met (e.g., housing, food insecurity, mental health) Develops and maintains client files to meet all local, state, and federal program requirements. Integrate harm reduction principles and strategies to mitigate health risks and improve patient outcomes. Other duties as assigned by the Nursing Manager.
