About the role
To identify, address and provide for the residents medically related social and emotional needs To uphold and ensure that resident rights, human dignity, and personal privacy are enhanced To prepare and assist residents with their plans for discharge To complete documentation required by state and federal regulations, and collaborate with families in order to meet the needs of each resident Responsible for compiling a list for the Eye Care Group and facility dentist for their assigned areas Make arrangements for and obtain needed adaptive equipment, clothing and personal items for assigned areas Respond to the needs of the facility, residents, and families in a timely and appropriate manner Is a member of the Interdisciplinary Care Plan team; completes the resident interview, assessment, and updates documentation according to state and federal requirements for assigned areas Orient new residents on assigned areas and families to the facility environment and facility policies and procedures Develop and complete a psychosocial assessment BIMS, PHQ-9 for each resident on assigned area Document Social Service progress notes in accordance with state and federal guidelines Actively participate in the interdisciplinary care plan conferences of assigned areas by identifying psychosocial issues on the resident's care plan and update changes timely Interpret, promote, advocate, and protect resident rights, privacy, dignity, and freedom of choice Assist with discharge planning services and complete discharge plan of assigned residents in accordance with discharge plan policy Complete the social service portion of the Minimum Data Set in a timely manner and communicate with supervisor if unable to complete assessments timely Conduct tours for persons interested in making admission in the facility when admission clerk is unavailable Complete entire admission process for new residents for designated floors in accordance with the admission policy if admission clerk is unavailable Complete annual resident review 77/78 and send copies to CMH and families/responsible parties Individual and/or group counseling to residents and families at times of adjustment, crisis or need. Assist staff in developing interventions and behavior plans for assigned areas Participate in room change decisions on assigned areas in accordance with the room change policy Identify community services and help the resident's families utilize them when needed Establish and maintain a resource file which reflects community agencies utilized for referrals Upon a resident's discharge to hospital Social Work staff to contact Case Manager seeking daily updates for documentation and continuity of care Promote facility programs and functions in the community; establish positive interaction within the community Assist residents on assigned areas in the decision-making process concerning their own health care and assist them with the appropriate forms as necessary, i.e., Patient Advocate Attends training programs and other meetings to remain knowledgeable concerning developments in the long term care field Keep Director of Social Services informed of departmental concerns Maintains a positive and professional attitude and contributes to the facility interdepartmental team Observe, record, and notify staff of changes in attitude, behavior, or personality with any resident. Other duties as needed or assigned by the Director of Social Services or Administrator
