Registered Nurse - CASE MANAGER - Utilization Management

Primarily Monday-Friday, Dayshift, Salaried. Discharge Planning Case Manager will apply disease specific and general clinical knowledge to assist in the coordination and implementation of a successful transition plan from the hospital setting. This position will assist in identification of continued clinical needs while taking into consideration the patient's functional level and current support system in place, in compliance with legal and regulatory requirements. Will use clinical decision support tools such as InterQual and MCG to help determine appropriate level of care and length of stay.
