Self Hired Support Staff (44215)

Assist persons with developmental disabilities to take part in activities of their choice, to give a reprieve to the parent/guardian/caretake from caretaking duties while the individual takes part in the program that fosters the opportunity of learning through athletics, arts, crafts, and community involvement. Responsibilities: - Provide door-to-door transportation to scheduled activities (if applicable) - Promote self-advocacy and choice -Assist individual in making informed choices -Promote self-determination and community inclusion -Keep the individual's aspirations the focal point of their actions -Take all reasonable steps to ensure the health and safety of the persons receiving services -Participate in the Life Plan meeting at least every six months (if applicable) - As outlined in the Staff Action Plan (SAP), assist individuals in acquiring, retaining and improving self-help skills socialization, and adaptive skills necessary in the home and community-based settings -Complete all services documentation under ADM# 2005 - 02 -Take all reasonable steps to ensure the health and safety of individuals
