The Opportunity: Silvergate Fallbrook, part of AmeriCare Health & Retirement, is seeking a compassionate and skilled Part-Time LVN to support our award ‑ winning senior living community. What You’ll Do: Provide clinical oversight and clear direction to caregiving staff Assess residents, document changes, and communicate with physicians and families Perform diabetic care, medication tasks, and follow physician orders Coordinate medical appointments and support proactive health management Partner with Home Health/Hospice teams to ensure comfort and continuity of care Train, coach, and support caregiving staff Uphold infection control and regulatory standards (Title 22)
