Enrollment Specialist, PACE Program, Full-time

Job Summary The Enrollment Specialist serves as the initial point of contact for prospective PACE Georgia participants and their representatives, guiding them through the enrollment process from first inquiry through program entry. This role is responsible for gathering demographic, medical, insurance, and financial information to determine eligibility; coordinating assessments, documentation, and Medicaid application support; and ensuring all deadlines and program requirements are met. The Enrollment Specialist builds and maintains positive relationships with prospective participants, families, referral sources, and Medicaid contacts, while partnering closely with the Interdisciplinary Team to facilitate communication, scheduling, and care plan development. Additional responsibilities include representing PACE Georgia at community presentations, health fairs, and outreach events to promote the program as a vital community resource.
