Director of Population Health

Volunteers of America National Services (VOANS) is seeking a Director of Population Health to lead and advance efforts to improve health outcomes for participants within VOANS PACE Organizations. This role will be pivotal in driving PACE quality improvements and managing costs while ensuring participant centered care plans for our participant population. Schedule: M-F 8:00 AM-5:00 PM Salary: $120,000 - $130,000 (Based on Experience) Travel: This is a FULLY REMOTE position and will travel to PACE programs as needed.
