Location: San Diego Central and East About The Opportunity The Speech-Language Pathologist (SLP) evaluates, plans, and provides speech therapy services to adult patients in their homes in accordance with physician orders and the Plan of Care (POC). Responsibilities • Perform comprehensive speech, language, cognitive, and swallowing evaluations. • Assess the home environment and recommend adaptive equipment as needed. • Develop and update individualized Plans of Care in collaboration with the physician and care team. • Provide skilled speech therapy interventions using evidence-based techniques. • Monitor patient progress and modify treatment plans accordingly. • Document visits accurately and timely in the electronic medical record. • Communicate patient status to physicians, case managers, and interdisciplinary team members. • Educate patients, families, and caregivers on therapy goals and home programs. • Participate in case conferences, in-services, and team meetings. • Follow infection control and HIPAA privacy standards. Required Qualifications • Active Speech-Language Pathologist license in the state of practice. • Master’s or Doctorate degree in Speech-Language Pathology. • Current BLS/CPR certification • Valid driver’s license, insured vehicle, and ability to travel to patient homes • Ability to work independently and manage schedule effectively • Experience working with adult/geriatric populations • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. Pay per visit ranges from $95 to $120 visit commensurate with experience and depending on type of visit.
