AFTERCARE CRISIS WORKER

Provide support, guidance, and counseling to Close to Home (C2H) youth during their time in the program and throughout the aftercare phase. Respond to crises in C2H residences and assist youth and families during aftercare to ensure safety, stability, and successful reintegration.
