Social Worker | Care Manager (MHRT/C or Eligible) - Adults

PCHC’s Care Management team is seeking a Social Worker/Care Manager (MHRT/C or eligible) to join our mission-driven, interdisciplinary care teams. In this role, you’ll build trusted relationships with adult patients, coordinate across providers, and connect individuals to essential community resources that support stability and well-being. This role is best suited for someone who is patient, flexible, and energized by navigating complex systems and barriers to care.
