Primary Care Provider MD/DO (Bilingual English / Spanish) Union Health Center – New York, NY Job description The Union Health Center (UHC) is seeking a full-time Primary Care Provider (Bilingual English/Spanish) to join our team! Background The Union Health Center (UHC) is an innovative primary and multi-specialty center focused on providing well-coordinated and accessible healthcare. UHC has been providing care to New Yorkers for over one hundred years to union members and their families. The center is built around a team-based model that integrates health coaches and serves a diverse patient population. UHC maintains Primary Care PCMH recognition and is certified as an Article 28 facility. Role Summary The candidate will be an integral part of a health care team providing excellent patient care directed toward improving quality outcomes and patient satisfaction. Reporting to the Medical Director, the candidate will work closely with committed staff in providing outstanding care to a multi-cultural population. Position Responsibilities: Delivers professional services within the Center. Elicits and records information regarding patient's medical history electronically. Examines patient to determine general physical condition. Orders or executes various tests, analyses, and diagnostic images to provide information on patient's condition. Analyzes reports and findings of tests and of examination, and diagnoses condition. Administers or prescribes treatments and drugs. Performs telemedicine including phone calls and video calls to patients and records findings electronically. Completes charting in a timely manner. Practices medicine in accordance with the federal and state guidelines. Advises patients concerning diet, hygiene, and methods for prevention of disease. Provides lab and diagnostic test results to patients in a timely manner/ Reports outbreak of contagious diseases to governmental authorities. Refers patients to medical specialist or other practitioner for specialized treatment. Performs minor procedures. Contributes to the development of policies and standards of patient care by participating in committees. Complies with the Union Health Center’s policies & procedures related to privacy and security of identifiable patient information (PHI), as explained in the HIPAA (Health Insurance Portability & Accountability Act) training. Supports the stated goals and objectives of the Patient Centered Medical Home by working cooperatively with all clinical and non-clinical staff to develop procedures that enhance the Health Center’s team-based approach to patient care.
