JOB SUMMARY The Prescription Assistance Program helps uninsured or underinsured patients obtain needed medications through programs offered by pharmaceutical companies that provide these medications at no cost to eligible patients. An employee in the Patient Advocate position is responsible for coordinating assistance applications for patients, acting as a liaison between various medical providers and pharmaceutical companies, advocating for patient concerns, and solving problems. JOB SPECIFICATIONS Responsibilities/Duties Assist patients with applications for medications for their completion/submission directly to pharmaceutical companies. Track pending receipt of medications. Sort and package medications for patient pick-up. Contact patients when medications arrive Distribute medications to patients. Document medications distributed to patients in RxAssist Complete refills for patients: Update data in RxAssist Track refill receipt/contact patients for pick-up Assess patients for Social Determinants of Health (SDoH) Other duties as assigned
