PD Rehabilitation Services Specialist I **On-call position**

**This is on-call as needed position that is 100% field based. The individual needs to understand the impact of behavioral health symptoms on presentation and importance of leveraging community and family supports. ** Plan and provide rehabilitation client service and other activities in the management of a client caseload through individual and group facilitation in a structured, therapeutic milieu, promoting client independence and community integration and contributing to the achievement of client goals and objectives.
