Case Manager - Inpatient Youth

Unison Health is seeking a Case Manager to support children and adolescents in our inpatient Children's Wellness Center (CWC). This role coordinates care with clients, families, and the treatment team, assists with discharge planning, connects families to community resources, and supports a safe and successful transition to ongoing care.
