Chaplain, Hospice, PRN

Job Summary The Chaplain serves as a member of the interdisciplinary team, providing pastoral and spiritual care to patients and families throughout the course of terminal illness. This role conducts spiritual assessments, develops individualized care plans, and coordinates with the team to address spiritual, cultural, and emotional needs. The Chaplain upholds patient confidentiality, honors cultural diversity and lifestyle differences, and supports the hospice philosophy, goals, and objectives in alignment with Hospice Savannah's values and service excellence standards.
