About This Role:
- Be a Steady Guide for Patients & Families
Provide clarity, compassion, and support during end-of-life care - when it matters most.
- Lead the Plan of Care
Ensure every patient’s care is aligned with their goals, focused on comfort, dignity, and quality of life.
- Collaborate Across the Care Team
Work closely with physicians, social workers, chaplains, aides, and caregivers to deliver holistic hospice care.
- Own the Clinical Picture
You are the hub - connecting referrals, providers, and care teams to ensure seamless, timely care.

What You’ll Do
- Coordinate and oversee hospice patient care across the interdisciplinary team, including contracted providers
- Serve as the central point of communication between physicians, caregivers, patients, and internal teams
- Manage physician orders and ensure timely entry, approval, and alignment with the plan of care
