Position Title: Social Work and Behavioral Health Manager Department: PACE Supervisor: Director of Clinical Services Hours per week: Full Time; 1.0 FTE FLSA Status: Exempt Pay Range: The pay range for the position is $90,000 - $120,000, commensurate with experience and licensing; including other factors, such as language, certifications, etc. Primary Function: The PACE Social Work and Behavioral Health Manager oversees the delivery of social work and behavioral health services within the Program of All-Inclusive Care for the Elderly (PACE). This position ensures high-quality, person-centered care by supervising social work staff, coordinating behavioral health resources, supporting interdisciplinary team (IDT) collaboration, and promoting participants’ psychosocial well-being, independence, and ability to remain safely in the community. Duties & Responsibilities: • Provide supervision, guidance, and performance management to social work staff, including hiring, training, and evaluation. • Ensure delivery of comprehensive assessments and individualized care plans for participants. • Carry a caseload of participants to support program operations. • Participate in IDT updates, care plan meetings, and ongoing care coordination. • Collaborate with the interdisciplinary team (IDT) to develop, implement, and monitor care plans • Support staff in addressing complex situations, including mental health, housing instability, caregiver stress, and end-of-life planning. • Ensure compliance with PACE regulations, documentation standards, and organizational policies. • Monitor quality outcomes and implement improvements related to social work services. • Facilitate team meetings and case conferences as needed. • Provide crisis intervention and support for participants and families. • Promote participant rights, dignity, and culturally competent care. • Identify and help present cases to the inter-PACE organization ethics committee. • Coordinate with community resources to support participants’ social, emotional, and environmental needs. • Participate in program development, audits, and accreditation activities. • Collaborate with behavioral health providers (internal and external) to support participant mental health needs. • Ensure timely and accurate EMR documentation and reporting.

Pay Range: The pay range for the position is $90,000 - $120,000
