Hoelter Vocational Training Center (HJH/VTC) , is seeking a compassionate and motivated Case Manager | Supportive Services Specialist to support individuals as they pursue vocational training and long-term employment opportunities. What You'll Do As a Case Manager, you'll work directly with participants to help them navigate challenges, connect with resources, and reach their employment goals. Key responsibilities include: Provide supportive, client-centered case management services. Assist participants in identifying and overcoming barriers to training and employment. Connect individuals with community resources and supportive services such as transportation, housing, healthcare, childcare, and other needs. Develop and maintain Individual Employment Development Plans (IEDPs). Monitor participant progress and provide ongoing encouragement and support. Collaborate with staff, employers, training providers, and community partners. Document services, case notes, and participant outcomes using electronic systems. Support compliance with program requirements, grant guidelines, and organizational policies. Participate in team meetings, training opportunities, and professional development activities.
