Behavioral Health Case Manager I

POSITION SUMMARY The Behavioral Health Case Manager is responsible for managing an assigned caseload of clients and assessing client needs by developing, implementing, and reviewing service plans, and working with other internal and external resources in meeting/achieving client service needs.
