Job Summary The Community Liaison builds and maintains relationships with community referral sources, healthcare and human services professionals, and senior‑focused organizations to promote the PACE Georgia program and support enrollment growth. This role increases program visibility through targeted outreach, collaborates with operations to align community engagement with organizational goals, provides information to prospective participants and families, and ensures timely documentation of referral activity in the CRM.
