Care Coordinator - Preventive Cardiology Clinic

The Care Coordinator will support the clinic and physician care team by providing administrative and patient care services. This is a full-time position working Monday - Friday 7:30 AM - 4:00 PM out of our Edina and Oakdale locations. Preventive Cardiology Clinic is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
