Transition Service Coordinator

The Transitional Services Coordinator is a member of the Rite of Passage Transition program and is responsible for the da y-to-day monitoring, contact a nd supervision of the youth assigned to the Transitional Services Program. Assists the students with school enrollment, reemployment, family reunification and transition into the community. This position reports to the Clinical director.
