Medical Case Manager

The Medical Case Manager is responsible for coordinating client assessments and facilitating access to comprehensive healthcare and support services. This includes referrals to medical, dental, mental health, and substance use services, as well as assistance with pharmaceutical access, treatment education, and other supportive services required by Care Resource clients.
