Location: Central Riverside Job Summary The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician. Qualifications • Graduate of an AMA approved (or equivalent) PT program • Licensed PT in the state of practice • 2+ years experience preferred • 1 year of home health experience required • Current CPR certification • Ability to work independently • Strong verbal and written communication skills • Valid driver’s license and insured vehicle • Meets pre employment health requirements Responsibilities • Provide PT services as ordered by physician • Assist with patient evaluations and Plan of Care • Document and report patient progress • Educate patients, families, and staff • Evaluate home environment as needed • Maintain accurate clinical documentation Why Join us? Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
