Clinical Case Manager (CMA/RMA or LPN) - Transitions of Care

Full Circle Health is looking for a Clinical Case Manager (LPN or CMA/RMA) to support our Transitions of Care Program across our clinics! The purpose of the Clinical Case Manager is to work collaboratively with the health care provider(s), interdisciplinary team, patients and community health partners to coordinate care for individuals with chronic or complex conditions, or those requiring high levels of care coordination. The Clinical Case Manager is responsible for assisting with Transitions of Care post hospitalization. Schedule 8:00am – 5:00pm, Monday - Friday daytime shifts.
