Scope of Position: The Critical Care Team is seeking a Licensed Clinical Intervention Specialist who will provide consultation, training, and clinical support to outreach professionals while delivering direct interventions as part of a multidisciplinary team. The role focuses on promoting the safety, stability, and well-being of older adults and medically fragile individuals who have experienced or are currently experiencing homelessness, ensuring that comprehensive, trauma-informed, and person-centered care is integrated into all aspects of service delivery. Essential Duties and Responsibilities : Provide consultation to outreach team staff across NYC, recommending and training on specialized interventions such as conducting relevant assessments; making referrals to community-based services (e.g., homecare and socialization programs); completing housing applications; advocating for benefits and entitlements; coordinating care and services; and assisting with community navigation, including accompaniment to behavioral health and medical appointments. Facilitate and lead case-specific conferences in community settings, including inpatient and outpatient behavioral health facilities, community-based social service provider sites, participants’ homes, shelters, and locations where unhoused individuals reside. Train staff and provide direct skill-building support to promote optimal mental and emotional health, including the use of preventive strategies to reduce medical risk. Engage individuals regarding substance use, mental health, medical concerns, family and social relationships, stress and symptom management, and medication management. Actively participate in multidisciplinary meetings. A willingness to travel throughout Manhattan, Brooklyn, the Bronx, and Queens.
