PHYSICAL THERAPIST - HOME HEALTH

Job Summary: The licensed Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff. Job Duties: Provide PT services prescribed by a physician, which can be safely provided in the home to include: a. Initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points (G333, G336). b. Developing and revising the plan of care in consultation with the physician and other care-team members (G162, G186). c. Providing therapeutic treatments and evaluation of equipment needs as applicable. d. Advising and consulting with the family and other agency personnel as applicable regarding medical equipment, HEP, safety Issues (G143, G188). e. Preparing clinical and progress notes (G187). f. Evaluating outcomes of care. g. Supervising Therapy Assistants and Home Health Aides as appropriate (G190). h. Planning for discharge. Will follow all policies and procedures including safeguarding the privacy of protected patient health information and the Compliance Program. Participating in in-services and QA programs as requested. (G189). All other duties as assigned.
