WISe Care Coordinator

The WISe Care Coordinator coordinates and facilitates the strengths-based and family-driven Wraparound with Intensive Services (WISe) process for trauma-impacted children and families. This position is the care coordinator and facilitator of the multidisciplinary Child and Family Team and is responsible for leading the team through the phases and activities of WISe. The Care Coordinator contributes knowledge and skills to ensure the team process honors each member’s role, responsibility, and perspective. Work is guided by high fidelity wraparound principles and provided in a team framework, in close collaboration with Akin’s peer partners, therapists, and natural and community supports.
